Please print and fill out the appropriate forms below. You may send them in via fax or email prior to your exam or bring them to your exam. If you are unable to do so, please arrive 15 minutes prior to your scheduled appointment time.
Appointments are required for all examinations, evaluations, and vision therapy. Appointments are recommended for all other services including selecting eye wear, frame repairs, frame adjustments, and order pick ups to be sure we are available to help you.
We request a minimum of 24 hours notice if you are unable to keep your appointment.
Vision Plans Accepted:
We participate with VSP, EyeMed, Medicare (not the Advantage plan), and most No Fault plans.
Invoices with the appropriate procedure and diagnosis codes are provided so patient's can submit to their medical insurance for out of network reimbursement.
Types of Payment Accepted:
We accept Visa, MasterCard, Checks, Cash, and Apple Pay at time of service.
